March 5, 2012 Medical Report HISTORY OF PRESENT infirmity: long-suffering was presented to ER with acute type AB pain in mightily quadrant, radiating to the back. Patient also has a hx of continuing pancreatitis and possible intoxicant abuse. .PHYSICAL EXAMINATION FINDINGS: The patient is telling female in her juvenile 30s. Blood tests were do and results were marvellous levels of lipids and too oft calcium. diagnose for this is, acute lipoidemia along with modest hypercalcemia. Because of her hx of hypertriglyceridemia, hypercalcemia, cystic fibrosis ,and history of chronic pancreatitis, a blood test was make to check the insulin levels as well. Results found a decreased insulin level and blood amylase levels were slightly elevated parallel with lipase concentrations. This led us to believe the patient is experiencing these symptoms because of chronic pancreatitis. A stethoscope was used to pick up to the abdomen and the physician note no bowel sounds.
